Craig Wisbiski's Bio
Craig has been in his current role as Purchasing/Inventory Control Manager since 2002 and is responsible for vendor relationships and selection, quality of products, maintaining inventory levels at all company locations and directing all inter-company equipment transfers.
Craig originally joined Form Tech in 1986, shortly after attending Architecture School at the University of Houston. Over his long career at Form Tech, Craig has held various positions in engineering, sales, and accounting.
Frank Smiser's Bio
Frank joined Form Tech in 1998 as Branch Manager of the company’s Grand Rapids, MI operation. For nearly 19 years, Frank managed several regions across Michigan and Ohio for the company. In 2016, he assumed his current role as Major Projects & Key Account Manager, where he is directly responsible for Form Tech’s largest and/or most complex formwork requirements. Frank’s technical expertise, grasp of overall project requirements, and years of field service experience has made him one of Form Tech’s top resources and a natural fit in his current role.
Frank began his career in 1984 with Greenrose Supply in Louisville, KY where he worked in an outside sales role for 6 years. He joined Symons in 1989 where he worked for 10 years, holding the positions of District Sales Manager, Steel Form Specialist, and finally as Dealer Sales Manager, just prior to joining Form Tech.
Frank attended the University of Louisville, and earned a Bachelor’s degree in Business in 1988.
Jeff Liedel 's Bio
Jeff joined Form Tech as Director of Engineering in June 2017, taking on the overall management of the Form Tech’s drafting/design department, including oversight of the process of creating job specific formwork application drawings, development of analysis tools and CAD standards, and working directly on complex projects. Jeff brings 20+ years of experience and practical problem-solving skills developed working on wide variety of construction projects.
In 1993, Jeff graduated from the University of Illinois in Urbana - Champaign, with a degree in Civil Engineering, with a structural emphasis. Then in 1997, he first became a licensed Professional Engineer (P.E.), subsequently adding additional locations, to a current total of licenses in eighteen states/jurisdictions.
Prior to joining Form Tech, Jeff has spent his entire career in various form of specialty structural engineering, including over twelve years in the realm of concrete formwork and shoring, with stints at WACO Scaffolding and Equipment, Safway Formworks and Symons.
Jim Upton's Bio
Jim joined Form Tech in 1990 as the Branch Manager of the company’s Charlotte, NC operation. Jim’s engineering capabilities coupled with his extensive formwork knowledge and field experience helped the company to expand rapidly in Charlotte and eventually led to the opening of the Raleigh, NC facility in 2004. Jim currently manages both the Charlotte, NC and Raleigh, NC operations for the company.
Prior to joining Form Tech, Jim worked for Lee Construction/National Engineering as a Project Engineer, and later, for Turner Murphy Construction as a Project Manager. He also worked for several years at Symons just before joining Form Tech.
Jim began his Civil Engineering studies at Central Piedmont Community College in Charlotte, where he qualified as a Phi Theta Kappa National Honor Society member. He subsequently earned his degree in Civil Engineering Technology in 1981 from the University of North Carolina Charlotte.
Jim Prager's Bio
Jim joined Form Tech in 2006 as a Regional Manager and is based in the company’s Pittsburgh facility. Jim is a proven and respected industry veteran and is highly competent in all formwork applications. In addition, he brings 30+ years of experience to the company’s Major Project Team.
Prior to taking his management role at Form Tech, Jim worked for George L. Wilson and Waco Scaffolding & Equipment in various outside sales roles for 20+ years.
Jim attended Slippery Rock State University and earned a Bachelor’s degree in Education in 1980.

Mark Kozlowski's Bio
Mark joined Form Tech in 2004 as Controller and manages the accounting functions for the company, across all locations, while also helping to drive continuous improvement within the department.
Mark graduated from Michigan State University in 1987 with a Bachelor’s degree in Accounting.
After graduating, Mark served as Controller for a division of CMI International, a large steel processing firm, for ten years. He then served for six years with SmithGroupJJR, one of the largest Architect/Engineering firms in the USA, where he was responsible for financial reporting and analysis while also managing the accounting functions for multiple locations.
Mike Fidelholtz's Bio
Mike has served as President of Form Tech since 1994 and continues to do so today.
Mike joined Form Tech in 1983 in an outside sales role at the Company’s Cleveland, OH facility. In 1985 he relocated to North Carolina and opened the Charlotte, NC operation. Later, in 1990, he moved to Form Tech’s headquarters in Michigan and became General Manager of the business. A few years later, in 1994, he and a former partner purchased the business from the founder of the company, Joe Kolar.
Mike attended Miami University (OH) where he earned a Bachelor’s degree in Communications in 1983.
Steve Zerio's Bio
Steve joined Form Tech in 2015 as CFO and is instrumental in helping the company define strategic actions and continuous improvement initiatives while also managing the financial aspects of the business.
Steve attended Wayne State University and graduated in 1983 with a Bachelor’s degree in Accounting. Shortly thereafter, Steve received his Michigan CPA Accreditation, and later, earned his MBA-Corporate Finance from Wayne State University in 1990.
Steve’s most recent engagement was with Anchor-Danly where he served as Senior VP of Operations & CFO for nearly ten years and was responsible for the manufacturing, accounting, IT, HR, and purchasing for 600 employees and seven plants throughout the USA and Canada.
Tim Doyle's Bio
Tim currently serves as the Regional Manager of Form Tech’s Detroit office, and is directly involved in projects throughout Michigan, NW Ohio, and Northern Indiana. He also participates in the Major Projects Group within the company.
Tim has many years of service with Form Tech, and is one of the most tenured people in the company today. He joined the company in 1981 in an outside sales role at the company’s original location in Detroit. After nearly 13 years, in 1993, he relocated to Pennsylvania and established the company’s operation in Pittsburgh, where he built the business for seven years prior to returning to Michigan in his current role.
Tim’s 35+ years of experience, field know-how and countless successes have made him a trusted advisor to many contractors throughout the region.
